Registration for the 2024-25 school year will begin on Tuesday, July 16!
You must complete all of the following in order to be fully registered and able to participate:

2. Physicals and Alternate Year Cards (BUFF Cards)
Students who are participating in a school-sponsored sport this school year and are in grades 5-12 must have an up-to-date physical, which is valid for two years. Physicals can be submitted online, delivered to the middle school office or delivered to the high school office. Physical forms are typically provided at your medical provider's office or they can be picked up at the middle school or high school offices.
A physical examination taken April 1 and thereafter is valid for the following two school years. A physical examination taken before April 1st is valid only for the remainder of that school year and the following year. Therefore, for the 2024-2025 school year, if your last physical was prior to April 1, 2023, you will need to schedule an appointment with your licensed medical provider to complete a physical.
If you are not due for a physical for the current school year, you must submit an Alternate Year Card (Buff Card), available HERE
NOTE: 5th and 6th grade students may participate in Cross Country and Wrestling and will also need to have a current physical on file.
3. Concussion Testing (SWAY)
All Unity Athletes in grades 7-12, including 5th and 6th graders who participate in Cross Country and Wrestling, must complete a SWAY Concussion Test annually. To complete the concussion test, you will need to download the Sway Medical App on the Apple App Store or the Google Play Store. The download is free and the test MUST be done on a cell phone or iPad. Once the app is downloaded, you will need to enter our Unity Athletics code, which is ACXDA. Lastly, you will fill in your name and other demographic and medical information. The test should take about 20 minutes to complete.
4. Pay Athletic Fees
High School Sports = $25 Each (Up to $100 Family Max)
Middle School Sports = FREE

Student athletes must abide by the Unity Athletic Code.
